The painting is hung on the wall at an angle, which indicates that the left part of the picture is equally as big as the right part.
While most of the players would just be saying that it makes no sense to speculate on what's on the left side, there have been previous experiences with From Software, where they did reveal an interesting secret through a painting.
In the summer fest trailer for Elden Ring, we saw a painting in the Volcano Manor, and in front, a crucible knight was standing. This picture was of Rykard, Lord of Blasphemy.
In the trailer, the painting was dark, and Rykard had snakes on his hand, while in the final cut release, the painting was brightened and the snakes were removed.
So, this indicates that the left part of the painting was smudged and darkened on purpose to hide a clear secret from the fans of Elden Ring DLC.

Going a bit into depth and putting the pieces of the puzzle together, let’s discuss another mechanic and secret in Elden Ring DLC Shadow of the Erdtree, through its trailer.
In the intro cutscene of the Mogh boss fight, after he comes out of the Cocoon beside Miquella, he takes the Cadaver’s hand. At that moment you can see that there is a ring on the finger, portrayed in the cutscene.
Why is this important if all of it is old or pre-DLC? Well, we know for a fact that the entrance to Elden Ring DLC - Shadow of the Erdtree is through this same location, so it becomes a place of priority.
Through the SekiroDubi Footage of the Ring on the hand in Elden Ring, we can see that it resembles three snakes curling and forming a ring.
Now, we have Miquella’s hand that takes us to the Land of Shadows, and it also symbolizes that the body inside the Cocoon isn’t of Miquella.
Miyazaki himself quoted that “The body has a strong relation to Miquella”, but he never said that it was indeed Miquella.
All of this takes us to Messmer, whose whole personality is based on snakes. A cursed and forgotten enemy. So, all of this could be Miquella’s plan taking place or something similar.
It could also be that the real body of Messmer is inside the Cocoon instead of Miquella. There are then some other observations about Messmer that are quite similar to what we have inside the game pre-DLC.
Looking at the Elden Ring DLC footage, we can see Messmer’s hand conjuring a black flame that turns blood red, and then into a fire.
These three phases are quite similar to Mogh the Omen before fighting, as he first turns into a black entity, and then slowly becomes blood red.
Also, there is quite a similarity between the blood flame and Messmer’s flame that he conjures. In the trailer for DLC, we also got footage of Messmer fighting, as he throws his blood that turns into fire.
All in all Messmer’s flame has a variety of every flame in the Elden Ring, including the black, red, blood, and even lava.
We already discussed the old man in the painting in Elden Ring DLC - Shadow of the Erdtree. That old man is the same person who is seen pulling golden bones out of his head. The mystery remained unanswered as to who he might be.
In the Elden Ring, the Elden Golds are a species of aliens who are from out of the Lands Between. These species are referred to as Numens, and as seen in the game itself, the power of Elden Gold remains in their bones.
All the Elden Gods are seen using their bones as weapons and so on. One example of this can be Radagon and Marika. When we defeat Radagon, he goes underwater and then transforms into an Elden Being, which is the final boss of the Elden Ring.
The weapon of this Elden Being - Final Boss is a spine-like sword made of bones. If you take a closer look at Radagon and Marika, there is an Elden Ring symbol inside them, which is also the spine made of bones.
So, it could be that the Old Man in the Painting in Elden Ring DLC - Shadow of the Erdtree is also an Elden God because the Golden Bones are coming out of the body.
The Old Man from the Painting could also be the Father of Marika in Elden Ring DLC - Shadow of the Erdtree. It could be the biggest secret in the Elden Ring DLC - Shadow of the Erdtree trailer so far.
She looks exactly like the Deathbed Companion whom we also talk with in Lands Between known as Fia. In Fia’s dress description, we find out that it is actually a Deathbed Companion's dress.
The woman in the painting of Elden Ring DLC is wearing the same Deathbed Companion dress. These companions also come from outside the Lands Between, from a distant land.
In the location where we find Fia and ants, there are eggs that break and then give you Numen Runes. These ants actually ate Numen’s and then in return when making sacks, we found lots of Numen Runes.
You also find the Elden Stars Incantations here that depict the birth of Holy Vassel and the Elden Ring. So, this area could well be the settlement of Numen’s.
The Deathbed Companions have a death wish, and the Golden Order destroys everything it lays its hands over. So, the connection between Elden Golds, Numen’s, Golden Order, and Deathbed Companions fit perfectly, as seen in the dark roots of the area.
